Sean McGinnis 140eaf453f Add issue link info to PR template 
While reviewing pull requests I noticed there are a lot of PRs that
are addressing open issues, but the PRs are not properly linked. This
results in a lot of issues being left open, even after the fix has
been merged.

This updates the PR template to give a short instruction on adding the
expected line in the PR description so that the issue and PR can be
linked automatically and closed on merge.

Use the right URL for Server Fault queries tagged Kubernetes, not the
wrong URL for Stack Overflow (which is more about workload / app
development help).

Update triage/support label references to kind/support 
The label triage/support has been reclassified as kind/support. The
kind/* family of labels makes more logical sense, as they describe the
"kind" of thing an issue or PR is.

This keeps the template short, and it lets Hugo use templating for
the current version.
